Aged Care Programs
Aged Care Programs provided by CHNC are the following:
The Community Visitors Scheme (CVS) is a national program funded by the Commonwealth Government.
The CVS aims to enrich the quality of life of residents of aged care homes who are socially isolated or lonely, and would benefit from a friendly visitor. The CVS also helps to establish links between people living in aged care homes and their local community. A community visitor is a volunteer who is matched with a resident of an aged care home and visits them regularly.
Community visitors will be able to visit your resident friend at least once a fortnight. Visits should be spent doing something you both enjoy- you might want to read, listen to music, chat and reminisce or watch a favourite TV program together. Sharing your time and your interests with the resident will help them feel better about themselves and enjoy their life more.
The program aims to maximise the ability of socially isolated older or frail aged Bankstown residents over 65 (including those from CALD backgrounds and other special needs groups), to continue to live independently at home and also assist in meeting their need for social contact.
If eligible, a potential client is assessed by the Social Support Worker and a support program is jointly developed for the client.
Chester Hill Neighbourhood Centre is now an active aged care provider for Home Care Packages.
A Home Care Package supports an older person with the day to day care to remain living at home and is designed to meet their specific needs.
Call MyAgedCare on 1800 200 422 to check your eligibility and to be assessed.
