Chester Hill Neighbourhood Centre is now an active aged care provider for Home Care Packages.
A Home Care Package supports an older person with the day to day care while living at home and is designed to meet their specific needs.
What is a Home Care Package?
As an Approved Aged Care Provider we will manage your package and individual budget, while helping with your goal of maintaining your independence.
To achieve this, we use a partnership approach. We establish this by working together with you and your family to ensure you have the right care and choice of support for you to continue to live at home.
